- Major Insurance Carriers
Many health insurance plans require patients to be pre-certified for service. This means you must obtain a referral from your primary care physician before you are seen by our physician. We will not be able to provide treatment unless we have proper documentation and approval from your health insurance company. We may need to delay, reschedule or cancel your appointment if you do not have a referral form.
To help us process your claim, please bring your insurance card to each visit.
Please take the time to print and complete the Patient Health Questionnaire and New Patient Information Record available on the website and bring them to your first appointment.
If we participate with your primary insurance plan, we will submit insurance claims for all services. However in most cases you are responsible for:
- Co-payments at the time of service
Unless your insurance plan covers office visits, you will be requested for full payment at the time of service. We accept cash, personal checks, credit and debit cards.
First time workman’s compensation patients must bring the following information:
- Date of injury
- Most recent work status
- All related medical records, x-ray films and office notes
- Medication/list of prescriptions
- Employer information (address, phone number)
- Insurance information (address, phone number)
All workman’s compensation claims must be verified by our office prior to your visit.
Although your injury may be work related, we will ask for your personal insurance information. If you refuse to provide us with your personal insurance as a back up and your workman’s compensation claim is denies, you will be personally responsible for payment.
